Part of our campaign to create the Lumber City History Center and Twin Cities Welcome Center includes the opportunity for residents and other individuals or organizations to help through the "5 & 10" Campaign. Your one time donation or pledge or regular donations, ranging from $5.10 to whatever you can afford, will help us turn the former G. C. Murphy 5 & 10 store building into the Lumber City History Center with the original appearance of the building as a 5 & 10 store, and the interior as a 1929 G. C. Murphy store as the setting for our North Tonawanda History Museum. Your help in any form, monetary donations, pledges, or pledges of donated services or equipment or materials for the project will enable us to purchase the building and convert it into the final completed project within a five-year timeline.

These printable MS Word Documents can be saved to your computer by simply clicking your mouse on the form title below, and choosing to save the file to your computer. From there, open the files in your word processor for viewing/printing.
